ADRC Transportation Program

The Aging and Disability Resource Center of Northwest Wisconsin provides non-emergency transportation to and from local and out of area medical appointments to individuals 60 years of age and older and disabled individuals of all ages, who reside in Burnett and Polk Counties. Transportation is coordinated through the ADRC of NW WI and rides are provided by volunteer drivers. Co-payments are charged to all riders, based on a reduced-rate fee. ADRC Transportation Program does not provide emergency transportation. Call 9-1-1 if you require emergency medical transportation and care.

Participants must be able to transfer into and out of the volunteer drivers vehicle with minimal assistance. We currently do not have a wheel chair accessible option for transportation. The volunteer driver program's primary focus is for medical trips but other types of trips will be considered on a case by case basis


Co-payments are based on the number of miles traveled by the volunteer driver starting from and ending at the volunteer driver's home. The Transportation Coordinator selects the available volunteer who is closest to the passenger's home toensure co-payments are as low as possible. Passengers are invoiced monthly based on the following rates:

Co-Payment: $0.29/mile

5 or More Appointments per Month: $0.14/mile

Trips to a VAMC (Minneapolis, St Cloud or Rice Lake): For veterans who receive travel pay, the cost is equal to the amount of travel pay they receive for that trip. For veterans who do not receive travel pay, the cost is $30 per roundtrip.

To Schedule a Ride:

Call the ADRC Transportation Reservation Line at 715-485-8592 or 877-485-2372 at least TWO BUSINESS DAYS prior to your appointment.

When you call please be prepared with the following appointment information:

  • Appointment date
  • Appointment time
  • Facility Name and Address

This information is required in order to enter your ride request in the dispatch program.

Once a driver has been assigned to your ride request, you will receive a call from the Transportation Coordinator. The volunteer driver assigned to the trip will contact the rider 24-48 hours prior to the trip to make final travel arrangements.

Transportation Program Use:

Medical appointments take precedence over other appointments or needs. We try to accommodate as many people as possible and understand there is a great need for public transportation to other locations outside of medical appointments. If we are able to find a driver willing to accommodate your transportation request we will accept ride requests outside of medical appointments.


The ADRC Van is used for socialization trips, shopping, errands and special events. This service is open to elderly and disabled individuals, living in the community, in Polk and Burnett Counties.

Trips for special events will be scheduled for each county. For the special trips our agency reserves the right to focus on the area of the county where there is the most interest and accommodate others as we are able. Local shopping/errand trips avaialble upon request. Call our office 5 days in advance to reserve a spot on the van for a special event or to schedule an individualized local shopping/errand trip. Please note for the individualized local shopping/errand trips we, the ADRC of NW WI, reserve the right to limit travel distance.

Details of the ADRC van service:

  • Co-payment of $1.00 per trip accepted at the time of service. Cash only please.
  • Local trips (in-town) to run errands available upon request.
  • Special event trips are scheduled and coordinated by the ADRC with special requests being considered as time and resources allow..
  • Trips will be advertised on the ADRC website and in The Voice.
  • Seats can be reserved by calling the ADRC 877-485-2372 at least 5 days in advance.


Van Schedule:

Local Shopping/Errand Trips

Transportation is available for local shopping and errands upon request. Contact the ADRC of Northwest Wisconsin at least 5 business days in advance to schedule the van. Please have a list of the locations you would like to shop at when you call to reserve your seat. Note: The ADRC of Northwest Wisconsin reserves the right to limit travel distance for individualized local shopping trips and this will be determined on a case by case basis.

The driver scheduled for your trip will call you the day before to let you know what time they will arrive at your house to pick you up.

ADRC New Rider Information

If you are interested in using the ADRC's Transportation Program but have never received a ride before, there are a few things you must complete prior to submitting your first transportation request or reserving a seat on the ADRC van.

Elderly and Disabled Transportation Program:

Call the ADRC of Northwest Wisconsin at 715-485-8592 or 877-485-2372 at least 2 weeks in advance to ensure all paperwork is complete prior to when you'll need transportation assistance. You will be required to complete a demographic information form. This form helps the ADRC gather basic contact information as well as financial information to determine if you are able to pay the co-payment. Financial disclosure is an options, however, non disclosure will result in the full co-payments being charged according to the prices listed previously. Once you are entered and approved in the dispatch program you are eligible to make ride requests.

The ADRC Social Van and Mini Bus:

The demographic information required to use our ADRC van is less and can be completed over the phone when you call to reseve a seat on the van for a shopping/errand trip or special event. Northwest Journey clients and other community members if ride request fits in schedule.
